Good Dogma is pleased to offer an ongoing, 6-week discount of 15% at Lemos Feed & Pet Supply to all our class registrants. Discounts are provided on dog accessories, training supplies, and Natura Brand dog food (EVO, Innova, California, Karma, Healthwise) for six (6) weeks, beginning with the first business day after our first class. In order to receive your discount, we must furnish LF&PS with your name, address, phone number, email, and dog's name. Please indicate your authorization below.

I AUTHORIZE GOOD DOGMA TO PROVIDE INFORMATION AND DEMONSTRATIONS IN OBEDIENCE TRAINING AND/OR BEHAVIOR MODIFICATION FOR ME AND MY DOG. I UNDERSTAND THAT THE OUTCOME OF THE TRAINING IS NOT GUARANTEED, THAT FEES ARE NOT REFUNDED, AND THAT I MUST WORK EVERY DAY WITH MY DOG TO ENSURE SUCCESS. I UNDERSTAND THAT ULTIMATELY I AM RESPONSIBLE FOR THE TRAINING OF MY DOG AND, AS THE LEGAL OWNER OF MY DOG, WAIVE AND RELEASE GOOD DOGMA FROM ANY CLAIMS OF INJURY, DAMAGE OR LIABILITY MY DOG MAY INFLICT AT ANY TIME.
